A50 YEAR OLD FEMALE PATIENTS WITH FEVER

CHEIF COMPLAINT :-
A 50 year old female bought to casualty with a complaint  of fever.

HISTORY OF PRESENT ILLNESS:-   A 50 yr old female patient who is agricultural  labour by occupation 
 was apparently asymptomatic  10days ago  and then she developed  high  grade fever continous fever with no h/o of chills . 
Fever associated  with cough and cold then she went to a local hospital were treatment taken and subsided. Then fever reappeared  3 days ago high grade fever associated  with  chills .

HISTORY OF PAST  ILLNESS : -
N/k/c/o Dm, hypertension,asthma epilepsy 
K/c/o  thyroid  since 12 years

PERSONAL HISTORY :-
Married
Appetite: Normal
Mixed diet
Allergies:-No 
Adequate  sleep
Menstrual history:-
Age of menarche: 12yrs
Age of menopause  : 45 yrs
Regular bowel movements 
Normal micturation

FAMILY HISTORY:
No history of diabetes , hypertension 
Asthma ,stroke.
 
GENERAL EXAMINATION : 
 Patient is conscious,coherent,coperative 
No history  of  cyanosis, clubbing
Lymphadenopathy, pallor, edema
Physical  status :moderately built
Adequate hydration

SYSTEMIC  EXAMINATION:
Cardiovascular system
Cardiac sounds: S1 S2+
Respiratory system
Dyspnoea: No
Wheeze: No
Shape Abdomen : scaphoid
DIAGNOSIS  :-
Dengue fever   thrombocytopenia 
INVESTIGATIONS:- 







TAB Dolo 650 mg
TAB pan 40 mg
TAB Thyronoma

CHIEF COMPLAINTS :    A 45 yr old male c/o fever since 5 days, abdominal pain , loose stools and vomitings since yesterday. HOPI: patient was apparently asymptomatic 5 days ago and then developed fever, which is low grade, not associated with chills and rigors and relieved on medication. He complains of diffuse pain abdomen since 11th October morning.
